    • If your primary care physician or specialist is concerned that you need an expedited dermatologic evaluation, your doctor may submit an “URGENT” referral request to Vital Dermatology (fax: 620-710-7703).

    • If you are experiencing a medical emergency, please call 911 or proceed to the nearest Emergency Department.

  • Please review the list ACCEPTED INSURANCES (updated 10/2024).

  • If you have an HMO plan (often listed on the insurance card), your insurance requires that your primary care physician fax an authorization to our office before a visit can be scheduled.

  • Note: Our office requires a credit card on file for all patients at check-in which is used in the event of a refund due, delinquent balance, or no show fee. An automatic no-show fee of 50$ is applied to appointments cancelled without a 24-hour notice.
