Our office requires credit card on file for all patients at check-in which is used in the event of a refund due, deliquent balance, no show fee. An automatic no-show fee is applied to appointments cancelled without 24 hour notice.
Prior to Starting, You Will Need:
Insurance cards (if applicable). Your appointment request will be declined without insurance information, unless you are self pay.
Access to your email (for security purposes, you will receive an email with a 4-digit security code to enter during scheduling).

Appointment Types: Skin cancer screenings, acne, rashes and hair loss require detailed, focused evaluations. A dedicated visit is required to thoroughly address these complaints. For example, if you have concerns about both acne and would like a skin cancer screening, we will address your primary concern at the first visit and schedule a follow-up appointment to address your additional issue.
Date of Birth: Select the small “triangle” next to the month and year to select the date
Insurance: If you cannot find your insurance in the pre-populate list, click the blue “ENTER AS TEXT” option
Appointment Note: If you have a SECONDARY INSURANCE, please include this information here or any additional information you would like our office to know. *NOTE: We cannot accept patients with a secondary of insurance of Medicaid.

APPOINTMENT CONFIRMATION REQUIRED: You will receive an initial email regarding your appointment REQUEST. However:
***YOUR APPOINTMENT IS NOT CONFIRMED UNTIL YOU RECEIVE AN ADDITIONAL CONFIRMATION EMAIL. If you have not received a second confirmation email within 3 business days, please text or call our office to confirm your appointment.***
