ROUTINE RETURN VISITS: Request an appointment via the ONLINE SCHEDULING ICON. This includes next available appointment openings. Examples: Visits for skin cancer screening, acne, hair loss, rashes that are not itchy or painful, or refills.
“URGENT” RETURN VISITS: Please CALL OUR PATIENT COORDINATORS at 850-888-2424 so that we can triage your appointment. Examples: Visits for rapidly growing/painful skin growths, itchy or painful rashes that are spreading, concern for a procedure-site infection.
If you are experiencing a medical emergency, please call 911 or proceed to the nearest Emergency Department.

Insurance cards (if applicable). Your appointment request will be declined without insurance information, unless you are self pay.
Access to your email. For security purposes, you will receive an email with a 4-digit security code to enter during scheduling).
HMO insurance plans: Our office will ensure you have an active authorization on file from your Primary Care Phyisician,

Date of Birth: Select the small “triangle” next to the month and year to select the date.
Insurance: If you cannot find your insurance in the pre-populate list, click the blue “ENTER AS TEXT” option.
Appointment Note: Include the follow information, if you have-
SECONDARY INSURANCE. *NOTE: We cannot accept patients with a secondary of insurance of Medicaid.
