Preparing for your Procedure

You are scheduled for an upcoming procedure at Vital Dermatology. Please review the following:

Pre-Procedure Screening Questionnaire

Cost-Estimate
-Payment is due at time of appointment for services performed and may co-pay, co-insurance &/or unmet deductible depending on your plan's benefits. A courtesy cost estimate can be provided.

On the Day of Your Procedure
-Take all your medications as usual.
-Avoid caffeine or alcohol.
-Bathe the morning of (or night before) as you will need to keep the procedure site dry for 2 days after the surgery.
-Arrive 5 minutes early to your appointment. You may drive yourself to the appointment.
-Plan for no heavy lifting >5 lbs for 2-3 weeks.

General Appointment Reminders
-Please arrive 5 minutes early.
-Call to reschedule if you have had confirm/suspect COVID-19 in the last 10 days or an active fever on day of the appointment.
-A credit card on file is required for all patients in order to process refunds and delinquent balances.
-If you need to reschedule/cancel, please give 24 hour notice or a no show fee of 75$ will apply.
