IMPORTANT INSURANCE ANNOUNCEMENT:
The nationwide insurance Clearinghouse, “Change Clearinghouse”, is responsible for the ability to verify patient insurance activity, eligibility and benefits. As a courtesy, our office verifies these insurance details prior to your visit. However, physician’s offices and pharmacies nationwide have been unable to access this Clearinghouse due to a cyber breach in Feb 2023.
WE HIGHLY ENCOURAGE YOU TO CALL YOUR INSURANCE TO (1) CONFIRM YOUR INSURANCE IS ACTIVE (2) DR. JENNA BEASLEY IS IN-NETWORK WITH YOUR PLAN (3) UNDERSTAND YOUR POLICY BENEFITS (DEDUCTIBLE, COPAY, COINSURANCE, OUT OF POCKET) AS PAYMENT IN FULL IS REQUIRED.
Insurance and Payment:
We do NOT accept patients with Medicaid as a primary or secondary insurance.
Credit card on file is required for all patients but is only utilized in the event of a patient refund, delinquent payment, no show or admin fee.
Bring your ID and insurance card(s). We encourage you to call your insurance company to ensure we are in your network.
Payment is due at time of your appointment for services performed and may include co-pay, co-insurance &/or unmet deductible depending on your plan's benefits. As a courtesy, we will attempt to verify your plan benefits, however, if your benefits cannot be verified, full payment based on a cost estimate is due at the time of service, and you will be refunded once your insurance has been processed.
Cash payments accepted, if exact change
HMO plans require an referral &/or a authorization for your insurance to cover your visit. Please contact your primary care physician’s office. If not received, you may be subject to self-pay pricing at the time of your visit.

*The standard in-office check-in is completed on an iPad. In order to maintain a limited lobby capacity, it is helpful to complete the Portal prior to your appointment

COVID-19 policy
Masks are optional, but may become mandatory in the event of a community COVID-19 surge.
Limit to 1 guest
Reasons to reschedule:
(1) Active symptoms of cold/URI even if you have had a negative COVID test. Symptoms include: fever, cough, sore throat, loss of taste/smell, diarrhea or body aches.
(2) Known COVID exposure within the past 7 days.
(3) Positive COVID test or suspected COVID in the last 14 days.
For your examination:
Remove all make-up prior to your appointment.
Do NOT wear lotion or moisturizer to your appointment.
Minors
A parent or legal guardian is required to accompany all patients 17 years old or younger. If a parent would like to designate another individual to bring the minor to his/her appointment, please complete the Minor Consent Form.

Lobby Hours
Lobby hours are closed from 11:20am to 12:10pm.
No Show and Late Appointment Policies
*No-Show Policy: In the event that you need to cancel or reschedule your appointment, please call 24 hours prior or a no-show fee of 50$ for a visit and 75$ for a surgery will be automatically charged to your credit card on file. If you No Show your first appointment, you may not be rescheduled.
*Late Appointment Policy: Patients who arrive more than 10 minutes after their scheduled appointment time will kindly be asked to reschedule.
